Assignment-Making, Skill Mix & Floating Rules

A good assignment is invisible — the strongest nurse gets the client most likely to crash, and the floated nurse gets the clients most like home
A sick call at 06:30 leaves Ruth one nurse short, one float arriving from labor and delivery, and a brand-new graduate on her second week.

The story
A sick call at 06:30 leaves Ruth one nurse short, one float arriving from labor and delivery, and a brand-new graduate on her second week.
Ruth has built assignments for twenty years and treats the assignment sheet like a safety device. Acuity drives everything: the most experienced nurse takes the least predictable client; the newest nurse takes the most stable, most predictable group; continuity counts, so yesterday's nurse keeps today's client when acuity allows. Floated staff are guests, not gap-fillers: a nurse floated to an unfamiliar unit receives the most stable clients, the ones most like their home unit, a named resource nurse, and never the charge role or the unstable admission. And when an assignment is genuinely unsafe, the sequence matters: voice the concern to the supervisor before accepting report when possible, state it objectively, and document — because leaving after accepting an assignment without handing it off is abandonment.
Story arc
The setup
Ruth redraws the board: five nurses, twenty clients, one float, one novice.
The turn
The float nurse from labor and delivery is offered the fresh cardiac admission 'because she's experienced.'
The crisis
The new graduate is quietly handed the unit's most complex client because 'the others are tired of him.'
The save
Ruth's final board: the senior nurse takes the complex client; the float gets two stable post-op clients like her home population plus a named buddy; the new graduate keeps her stable group from yesterday, with Ruth rounding twice.
